Think of a veterinary clinic that launched a wellness plan. Monthly fee, a set of covered visits and vaccines, and a discount on everything else. Sign-ups were strong.

A year later, the owner wanted to know if it was actually working. That's the right question, and it takes a few numbers:
• Active members and monthly plan revenue.
• What covered services cost to deliver, not just their list price.
• Whether members come in more, accept more care, and stay longer.
• Cancellations each month.

A good plan smooths cash flow and builds loyalty. A poorly priced one quietly gives away the care you'd have been paid for. The only way to know is to measure it.

Picture two dentists who have practiced together for years. They split profit evenly from day one, and it worked, until one of them cut back to three days a week.

Nothing was wrong. The formula just hadn't been revisited since the partnership started.

Most partner pay plans fall into three shapes:
• Equal split: simple, and it works when effort and production stay close.
• Production-based: each partner's pay follows what they produce and collect.
• Hybrid: a base for shared ownership and management, plus a share tied to production.

None is right for everyone. What matters is choosing on purpose, putting it in writing, and setting a date to review it, before a change in schedule makes the conversation personal.

💳 The average family health premium just hit $28,303 a year (KFF 2026 Employer Health Benefits Survey, Oct. 22). Workers pay $7,471 of it, up 9%. At firms under 200 workers: $9,687.

Practice owners, three things to review before renewal:
1️⃣ Employer-paid coverage isn't taxable pay to the employee (IRC §106(a))
2️⃣ HSA-qualified plan? 2027 HSA limits are $4,500 / $9,000 (Rev. Proc. 2026-24)
3️⃣ Under 50 full-time employees with no group plan? A QSEHRA reimburses up to $6,450 / $13,100 for 2026 (IRC §9831(d))

Consider a physician who refinanced a practice loan a few years after opening. The rate was better and the payment was lower. Everyone was happy.

What nobody revisited was the personal guarantee from the original loan, the equipment financing, and the office lease. Three separate documents, each one putting the doctor's personal balance sheet behind the practice.

That isn't unusual, and it isn't always avoidable. But it should be known:
• Keep a one-page list of every guarantee you've signed, with the lender and amount.
• Ask at every refinance or renewal whether the guarantee can be limited or released.
• Factor guarantees into the bigger plan, including bringing in a partner or selling.

The debt is the practice's. The guarantee is yours.
