Posted this last year but the list remains the same this year, and is one of the questions I am asked most so reposting it again!
What to bring your annual tax preparation appointment.
What you want to bring is the following: Some or most of these may not apply
If you are a new client you will want to bring a copy of your 2013 tax return
If you're a salaried employee then you'll have W2s
If you received over $10 in bank interest then you should have 1099-Int forms from the bank for this income
If you had dividends from stocks then you'll have 1099-DIV forms
If you had IRAs then you may get a form showing the value of your IRA at the end of the year, bring that.
If you had withdrawals from retirement accounts you'll have 1099-R forms
If you have children and had child care bring that information including the providers ID #, address, and phone number.
If you have rental property bring a profit and loss statement for each piece of property
If you sold securities in the previous year bring your broker statement and also bring information on acquisition dates and costs basis
If you're self employed bring a profit and loss statement for your business. Bring information on total miles driven for the year and total business miles driven. Bring information on your home office if you have one for self employment.
Any other income you had in '14(unemployment, gambling, alimony, health care reimbursements, jury duty, or any other 1099s you received, Schedule K1s - bring those)
Education Payments - if you or a dependent received a form 1098-T for tuition bring that.
If you're itemizing then you'll want to bring forms 1098 and mortgage interest statements, the amount you paid for state and or local taxes in 2014(usually from your W2, but if you owed the state money on your 2013 return bring that info too), if you purchased a vehicle in 2014 bring the sales tax paid figure for that, if you purchased a home in 2014 bring your HUD-1 closing statement. Bring information on property taxes paid in '14. Charitable contributions. Unreimbursed employee business expenses - tally those up and bring that info.
