D2391 and D2392 depend on connected surfaces, and miscoding can trigger denials or refund requests.
D2391 is for one posterior composite surface, while D2392 is for two connected surfaces in one preparation. A continuous mesial-occlusal lesion is one D2392, not two D2391s. Separate, non-contiguous restorations may be coded separately. Plans may apply an alternate benefit, paying the amalgam fee and leaving the patient responsible for the difference. Document the tooth, surfaces, date, and radiographs.
