* If any of your information has changed, please be sure to let us know or fill out a new information sheet. You can scan the QR code on our page, download a copy from our website, or pick one up at the office.
* Appointments are limited to Wednesdays in February ONLY. We encourage you to use our fast and convenient drop-off service.
* Please bring your Driver's License if you had it renewed in the past year
* If you had insurance through the MarketPlace, please bring us your 1095A form
*We will be open Saturdays in FEBRUARY ONLY from 9am until 12 pm.
** PAPERWORK DROPPED OFF AFTER MARCH 3RD WILL BE SUBJECT TO AN EXTENSION BEING FILED**
